The PlanWell Health Mobile App (App) is a technology purchased by employers to support efforts to assist their employees to lead healthier, more productive lives.

The App is ONLY available for purchase through employers. All user name and password information MUST come from your employer. If you do not have information regarding how to login, please contact your employer.

AFTER you have login credentials, if you experience problems downloading or accessing any portion of the app, please call 888-217-4769.

The PlanWell Health customer service team will ONLY answer questions regarding accessing the app and ensuring you are able to login via your mobile device. All other question should be directed to your employer
