Wellbridge Health offers an innovative, interactive chronic disease management program designed to improve quality of life, reduce avoidable emergency room visits and hospitalizations, improve self-management of chronic disease(s), and lower healthcare spend.

The BridgIT health app enables customizable remote symptom monitoring. Participants use the app to report on their health status daily to their dedicated, master’s-level health coach. The app is complemented by weekly, HIPAA-compliant, video chat-based health coaching sessions.
