The IntelliH patient App will allow you to conveniently engage with your Care team from the convenience of your home. IntelliH can only be used by you if your care provider has enabled you to use this and you are in their care. You should not make medical decisions unilaterally based on the vitals you acquire in IntelliH but rather to discuss the relevance of the data with a health care provider before taking appropriate clinical action.

IntelliH is an invite only App. Ask your provider if they are using the IntelliH solution to take care of patients at home.


HOW IT WORKS

Your Care team will send you an invitation to participate in the IntelliH Remote Patient Monitoring program. This invitation will include a unique one-time code to onboard you. Your care team will also provide you with one or more Vital monitoring device(s). Using the code you will be able to set your access credentials and use the IntelliH App, pair your devices to the App and then take your vitals. These vitals are uploaded to your care team's dedicated IntelliH portal in the cloud.


With this APP you can perform the following functions:

• Measure and send Vitals: Using the devices provided to you by your care team take your vitals. You can review your vitals if you are logged in. Your provider can see the vitals you have taken and can engage with you when necessary.

• Care Plan: Your care team will send you a daily care plan that informs you the vitals and the medications that you are required to take. The App will remind you to take your vitals and medications.

• You can interact with your care teams via secure Text, Health tweets as well as high-definition Video.

• Track Diet – You can track your diet by taking a picture of your food plate and identify the items and portion size. Your care team can review your diet as necessary.

• Track Activity - You can sync your activity data using Apple Health. The app reads step count and walking distance from HealthKit to help track your daily activity levels.

• Your Care team will list the medications that you have to take. You can inform your care team the medications that you have taken through a single tap. IntelliH helps to manage your medications. IntelliH tracks compliance through reminders as well as alerts.
