LeapConnect™ is a patented platform to help with caregiving every day and in urgent situations. The goal is to help loved ones maintain their independence and to bring insight, information and support tools to family, friends and care professionals.

LeapConnect™ provides real-time, remote monitoring of health and activity, using everyday devices to connect families and caregivers.

We help identify and notify loved ones and care team members of certain potentially concerning issues. Then, we help paint a picture of a loved one’s situation with meaningful context and insight.

If you are unable to get a hold or your loved one or they are not picking up the phone, our patented BreakThru™ feature allows permissioned family or members of a care team to try and reach them without requiring them to answer the phone.

LeapConnect’s suite of communication tools make it easy to securely share relevant information with both the care recipient and other members of a care team via integrated group chat, secure messaging or video conferencing.

Once a situation is being looked into or once that situation is resolved, other members of the care team are proactively informed.

To help with daily support, LeapConnect has both reactive monitoring and pro-active remote engagement tools. LeapConnect can help monitor for medication adherence; ensuring a loved one is where they are supposed to be; and lookout for abnormal device measures from connected devices that provide information on heart rate, activity level or other health measures. As well, LeapConnect offers custom check-in features to track behavioral responses to wellness indicators, chronic issues, or treatment programs and to remind a loved one to take their medications. LeapConnect provides a remote reminder and calendaring system to help coordinate their day and is designed to integrate third party service providers, to make ordering relevant services like transportation and meals, a seamless point-and-click experience.
