The tool allows users to collect data and generate reports from patients, family members, providers, and learners to help care teams receive real-time actionable feedback and improve team functioning. Team performance can be analyzed over time and in various settings. Data generated from the app will enable ongoing development of national benchmarks for team-based collaborative practice.
