The Kodiak Trials App is a tool to support the clinicians and staff participating in the Kodiak clinical trial program. This tool will allow certified users to access key trial data and protocol information for the Kodiak trials that their site is participating in. More specifically, users will be able to:

• Keep a real-time pulse on how many patients are randomized in each trial, per month and overall, as well as where they are in the treatment timeline.
• More easily look up inclusion/exclusion criteria to help identify eligible patients.
• Look up key protocol information, such as trial design, Schedule of Activities, and other trial-specific details to help keep randomized patients on track.
• Be notified of important reminders, such as protocol amendments, trial progress, etc.
Easy access to site monitor and contact information.

Here are some of the core features:
• Key protocol information for the following studies: Phase 1b (wAMD, DME, RVO), DAZZLE (wAMD), GLEAM and GLIMMER (DME), BEACON (RVO), DAYLIGHT (wAMD), and GLOW (NPDR).
° Brief overview of each study with study designs.
° Easy access to Inclusion, Exclusion and Retreatment criteria (where appropriate) for each study.
° Easy access to Schedule of Activities and IRT entry requirements.
• Randomization progress for each study.
• Subject progress and visit history.
• Quick access to email and phone numbers of Kodiak staff.

Note: access to the app is tightly controlled. To access the app, please contact your Kodiak Monitor for additional information to gain access to the closed group.
