May securely share their medical records with the Providers, screening results and log vital signs for immediate diagnosis and treatment plan.

Receive periodic check-ins and let your providers know how you are doing. Ability to manage and share tasks and goals with care team members. Timely reminders, alerts and requests for action will ensure that you maintain all your goals with ease.

Receive alerts for medication ingestion and refills. Missed medications will automate refill and appointment reminders, request prescription refills.

Avoid missing goals and event reminders by receiving push notifications. Contact your healthcare provider through secure messaging.

Flexibility to use during “Selfcare” with or without your Care Team. Continue to track your goals and manage your wellness. Stay focused, create positive habits, and build a long-term wellness plan. Keep an eye on the progress of your health and overall well-being.

Track Triggers:
The NixitHub app uses GPS location tracking to engage your care team members in helping you with your recovery plan. Location services must be enabled and the use of either cellular data or Wi-Fi when you are checking-in to locations is required for the mobile app to successfully support you. Use of GPS running in the background may decrease battery life.

Must be 18 years or older on day of sign-up.
