Cloud Forest Solutions' Story Mobile is designed to empower a coordinated care-team effort by aggregating and displaying up-to-the-moment, real-time information about a patient’s health and well-being, in multiple ways, and to a variety of care team members.
The Story Mobile application is a health management input/display application that aggregates elements of a patient’s health story collected from individual patient input, EHR, Hospital, Payer, and other data sources. The application presents comprehensive, up-to-date, contextual data. The Story application is intuitive, promotes self-management, and allows collaboration and care coordination to positively impact outcomes for all populations, including - pediatrics, chronic disease, acute care, complex care, and aging.
