Looking for a little extra motivation? CCP Motivate is an app supporting the Motivated Mondays program for members of Concierge Choice Physicians.

Each month, you’ll tackle a different mini-resolution like abstaining from sugar, limiting screen time or challenge yourself to add a positive behavior, such as increasing exercise or developing gratitude. At the end of the year, you will hopefully have kicked some bad habits and developed a few good ones too!

The CCP Motivate app keeps you informed and helps you track your progress, so download the app and get started!

### HOW TO GET STARTED

- Download the app and enter your Invite ID (you’ll get this from the CCP team).
- Sync data from the Health app in one easy step.
- Finish creating your account and say hello to us so we know you made it!
